I (You) hereby give my full consent to Ben Painter & Way of Life Tattoos to tattoo me. I have read and understand the information provided to me about the tattoo procedure, and I agree to all of the terms and conditions.

I am over the age of 18 years old, and I am not under the influence of drugs or alcohol. I have requested this tattoo of my own free will in accordance with the Tattooing of Minors Act 1969 – This information is true to the best of my ability.

I understand that there are risks associated with tattooing, including but not limited to infection, allergic reaction, and scarring. I have been informed of these risks, and I accept them.

I release; Ben Painter; Way of Life Tattoos and their associates from any and all liability for any injuries or damages that I may sustain as a result of the tattoo procedure.

I hereby acknowledge and understand, that I have been informed of the risks associated with getting a tattoo, including blood poisoning (Septicaemia), Scarring, Allergic Reactions, Localised Swelling/Trauma.

I understand that this procedure is a permanent mark for life to my skin and body and that no form of anaesthetic will be used in the procedure.

I understand that every care will be taken in a hygienic way including the use of pre-sterilised single-use needles.

I understand that a new tattoo is susceptible to infection until healed and that the care of the tattoo once I leave the studio is my own responsibility. I will follow the after-care procedure as explained and given to me in writing.

I hereby release Ben Painter & Way of Life Tattoos from any and all liability arising out of or connected with the performance of this procedure.

I, [Your Name], hereby give my full consent to the tattoo being carried out by the aforementioned practitioner. I have been informed of the potential complications of tattooing, including infection, swelling, and allergic reactions. I have also been given a written aftercare sheet containing more detailed information. I agree that it is my responsibility to read and follow the aftercare instructions until the tattoo has healed.

I confirm that the information I have provided on this consent form is correct to the best of my knowledge. I am over the age of consent for this procedure, as explained by the practitioner.

I understand that tattoos are permanent and cannot be removed. I am aware of the risks associated with tattooing and I voluntarily consent to the procedure.

I indemnify and hold harmless the Tattoo Shop against any claims, expenses, damages, and liabilities.
